Registration Process, Payment, & Waiver (Read the waiver below and indicate agreement on the registration form)
This on-line registration form may be used instead of mailing or emailing a completed registration form as found on the main Kauai Youth Conference page. You will be presented with a confirmation message upon successfully submitting your on-line registration. All registrations, including those done on-line, should be submitted by no later than 5:00pm on Friday before the event (i.e. January 29, 2010). In order to complete the on-line registration, you must by law be 18 years of age or older.
PAYMENT OPTIONS: Participants registering via this on-line form (as well as those using the standard form) may pay in advance by credit card using the Pay Now button below. Credit card transaction processing is provided by PayPal. Alternatively, you can pay in advance by check sent via standard mail, or at packet pickup with cash or check. All checks payable to Pu`uwai Canoe Club. If sending a check via standard mail, address to Pu`uwai Canoe Club, 6590-A Pu’upua Road, Kapa`a, HI 96746. Please note that credit card payments will NOT be accepted at the event, and must be received via this website prior to the deadline noted above.

WAIVER AGREEMENT: READ THIS DOCUMENT CAREFULLY. THIS WAIVER AGREEMENT WILL AFFECT YOUR LEGAL RIGHTS, AND WILL LIMIT OR ELIMINATE YOUR ABILITY TO BRING A FUTURE LAWSUIT. In consideration of being allowed to participate in the Kauai Youth Conference, the undersigned fully understand that they will be engaging in activities that involve risk of serious injury, including permanent disability and death, and severe social and economic losses which might result not only from their own actions, inactions or negligence, but the actions, inactions or negligence of others, including the rules of play, condition of premises, and/or equipment failure. They assume all the foregoing risks and accept personal responsibility for the damages following such injury, permanent disability or death, or damaged property. Additionally, they hereby release, waive, discharge and understand that they may not sue Hui O Mana Ka Pu`uwai Outrigger Canoe Club, its members, sponsors, coaches, event volunteers, employees of the organization, other participants, or owners/leasers of the premises used to conduct the event, hereinafter referred to as “releasees”, from demands, losses and damages on account of injury, including death or damage property, caused or alleged to be caused in whole or part by the negligence of the releasees or otherwise. Finally, they release, waive, discharge, and convey all rights to any photos, video, etc. taken at this public event to Hui O Mana Ka Pu`uwai Outrigger Canoe Club to use at their discretion for advertising & purposes. The undersigned have read the above waiver and release, understand that they have given up substantial rights by signing it and sign it voluntarily. All participants, as well as their responsible parents/guardians where indicated, have:

When signing the waiver, please provide:
initials of the participant over 18 years of age, or
