TriKauai On-line Competitor Registration

STEP 1: WAIVER
Please read the registration process information and participant waiver agreement shown to the right.  You will be asked to electronically sign the waiver upon entering your registration.
 
STEP 2: ON-LINE PAYMENT VIA PAYPAL
If you are paying on-line via credit card, please use the Pay Now option provided on the right side of this page.  Be sure to enter the participant name(s) associated with your payment in the fields provided. You may enter multiple payments, 1 for each registration, if you plan to complete multiple registrations at the same time. Complete all your payments before proceeding to on-line registration in STEP 3 below.

TriKauai Registration Process, Payment, & Waiver

This on-line registration form may be used instead of mailing or emailing a completed registration form as found on the main TriKauai page.  Please be sure to provide information for all participants (1, 2, & 3) if entering the triathlon as a team.  If you are entering the triathlon as an individual or as part of the fun run/walk, provide information for participant 1 only. You will be presented with a confirmation message upon successfully submitting your on-line registration.  All registrations, including those done on-line, should be submitted by no later than 5:00pm on Friday November 19.  However, we will be accepting registrations for the Fun/Run Walk at packet pickup - Saturday November 20, 12:00 noon to 5:00pm at ???Registration fees include a race t-shirt for every participant if registration is received by the deadlines above. Register early to guarantee that you receive a shirt on race day. In order to complete the on-line registration, you must by law be 18 years of age or older.


PAYMENT OPTIONS:  Participants registering via this on-line form (as well as those using the standard form) may pay in advance by credit card using the Pay Now button below.  Secure credit card transaction processing is provided by PayPal. Alternatively, you can pay in advance by check sent via standard mail, or at packet pickup with cash or check.  All checks payable to Pu`uwai Canoe Club.  If sending a check via standard mail, address to Pu`uwai Canoe Club, P.O. Box 1593, Kapa`a, HI 96746.  A $5 discount is available for members of the military competing in the triathlon as an individual. If you qualify for this discount, please be sure to select the appropriate option if paying via credit card below and be prepared to present your military ID at packet pickup.  "Early bird" registration is available through November 12, 2010.  Triathlon fees shown below will be increased by $10 after this date.  Please note that credit card payments will NOT be accepted at packet pickup, and must be received via this website prior to the deadline noted above.

 

When using the Pay Now option, please be sure to enter all participant names covered by the payment.  This will help us match your payment to your registration.

WAIVER AGREEMENT:  PLEASE READ THIS DOCUMENT CAREFULLY. THIS WAIVER AGREEMENT WILL AFFECT YOUR LEGAL RIGHTS, AND WILL LIMIT OR ELIMINATE YOUR ABILITY TO BRING A FUTURE LAWSUIT.

 

Hawaiian Canoe Racing Association Insurance Release of Liability

April 1, 2010 to March 31, 2011

 

In consideration of being allowed to participate in any way in Hawaiian Canoe Racing Association and its member organizations’ athletics/sports programs, and related events and activities INCLUDING THE TRIKAUAI TRIATHLON, the undersigned agrees and acknowledges:

 

1. THAT “PARTICIPANT” MEANS EACH INDIVIDUAL LISTED ON THIS FORM PARTICIPATING IN HAWAIIAN CANOE RACING ASSOCIATION (HEREINAFTER HCRA) ACTIVITIES. THE “UNDERSIGNED” MEANS EACH PARTICIPANT, AND WHEN A PARTICIPANT IS UNDER AGE 18, IT INCLUDES SUCH PARTICIPANT’S PARENT OR LEGAL GUARDIAN SIGNING ON BEHALF OF HIMSELF/HERSELF AND ON BEHALF OF THE MINOR PARTICIPANT. THE UNDERSIGNED UNDERSTAND AND AGREE THAT TAKING PART IN THE ACTIVITY, RACING, COMPETING, CANOEING AND USING HCRA FACILITIES, INCLUDING ON WATER AND ON LAND, FOR ANY PURPOSE (HEREINAFTER THE “ACTIVITY”) CAN BE HAZARDOUS AND PRESENTS A RISK OF PHYSICAL INJURY OR DEATH.

 

2. That the undersigned expressly ASSUME ALL RISKS associated with Participant’s participation in the Activity, known or unknown, inherent or otherwise; the Undersigned agree and understand these risks include but are not limited to risks associated with: accidents which occur during transportation or travel to and from location of Activity; slips and falls while getting into or out of the boats; overturning of the boats; loss of balance; impact, collision with or entrapment by trees, logs, deadfall, boats, equipment or other participants in the Water Activity; hypothermia; drowning; changing and inclement weather conditions including storms, high wind, high waves, and lightning; encounters with domestic or wild animals; negligence on the part of other participants; and negligence on the part of the releases, including the failure on the part of the releases to safeguard or protect me from the risks, dangers and hazards of participating in the activity.

 

3. TO HOLD HARMLESS, RELEASE, DEFEND, AND INDEMNIFY Hawaiian Canoe Racing Association, its member associations, its affiliated clubs, their respective administrators, directors, agents, coaches, and other volunteers or employees of the organization, other participants sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (each hereinafter a “Released Party”) FROM ANY AND ALL LIABILITY and/or claims for injury or death to persons or damage to property arising from the Participant’s participation in the Activity. The Undersigned take full responsibility for any injury or loss to Participant, including death, which Participant may suffer, arising in whole or in part out of the Activity, INCLUDING THOSE INJURIES AND DAMAGES CAUSED BY ANY RELEASED PARTY’S ALLEGED OR ACTUAL NEGLIGENCE OR BREACH OF ANY EXPRESS OR IMPLIED WARRANTY. By execution of this release, THE UNDERSIGNED AGREE NOT TO SUE A RELEASED PARTY and agree they are releasing any right to make a claim or file a lawsuit against any Released Party. The Undersigned further agree to defend and indemnify each Released Party for any and all claims of the Undersigned and/or a third party arising in whole or in part from the Participant’s participation in the Activity. The Undersigned agree to pay all costs and attorney’s fees incurred by any Released Party in defending a claim or suit brought by or on behalf of the Undersigned.

 

4. The undersigned parent or legal guardian acknowledges that he/she is also signing this release on behalf of the minor Participant, that he/she is WAIVING CERTAIN RIGHTS ON BEHALF OF THE MINOR PARTICIPANT that the minor Participant otherwise may have and that THE MINOR PARTICIPANT SHALL BE BOUND BY ALL THE TERMS OF THIS RELEASE. By signing this agreement without a parent or guardian’s signature, Participant represents they are at least 18 years of age, or, if signing as the parent or guardian of the Participant, signer represents they are the legal parent or guardian of the minor Participant. THE UNDERSIGNED AGREES TO INDEMNIFY THE RELEASED PARTIES FOR ALL LIABILITY AND CLAIMS, INCLUDING ATTORNEYS FEES, ARISING FROM ANY MISREPRESENTATIONS MADE IN THIS APPLICATION OR FRAUDULENT EXECUTION OF THIS AGREEMENT.

 

5. Participants release, waive, discharge, and convey all rights to any photos, video, etc. taken at this public event to Pu`uwai  and Kukui`ula Outrigger Canoe Clubs for use at their discretion for advertising & other purposes.

 

THE UNDERSIGNED HAVING READ THE ABOVE WAIVER AND RELEASE, UNDERSTAND THAT THEY HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT AND SIGN IT VOLUNTARILY.

 

All participants, as well as their responsible parents/guardians where indicated:

  • acknowledge the above waiver by selecting the I agree to this waiver check box, and
  • confirm their signing of the waiver agreement by entering their initials or full names

 

When signing the waiver, please provide:

  • initials of the athlete over 18 years of age, or
  • full name of the parent/legal guardian of minor athletes under 18 years of age, or
  • full name of the parent/legal guardian of incapacitated/mentally challenged athletes.
